Adopt a Family Adoptee Application

    Welcome to the Gifts of Kindness Adopt a Family program.

    If you are facing financial difficulties, this program will match you with a sponsor who will provide your family with gifts during this holiday season. Please complete this application completely to apply for sponsorship.

  3. How It Works

    1. Application & Intake Screening: Complete this application and a staff member from the City's Family & Community Resource team will be in touch to do a detailed intake assessment to ensure you qualify for the program.

    2. Getting Matched: After you have been accepted into the program, your contact information and wish lists will be shared with a sponsor (usually in early December).

    3. Receiving Delivery: A sponsor will be in contact to arrange a contactless delivery of gifts before December 18, 2020. Sponsors are being asked to following COVID-19 door-to-door safe practices (found here).

  4. Sharing Personal Information

    In order to allow the sponsor to know what to purchase and connect with you to arrange a drop off, we request that we share all the information below with the sponsor you are matched with. Please confirm that you understand and accept that we will be sharing this personal information. If you have any questions about this, please email crcinfo@chestermere.ca prior to finishing this form.

    Please note that you are welcome to suggest gifts for the whole family, however, please note that children and seniors will be prioritized and adult family members may or may not receive gifts from a sponsor.

    After you have completed this form and submitted it, a member of the Family & Community Resource Centre (FCRC) Team will be in contact to discuss your application. You can expect a call in the month of November. Please note that the Adopt a Family program is subject to participants and there is no guarantee of matches.     The personal information is being collected for activities related to accepting volunteers for the Gifts of Kindness Adopt a Family program being coordinated by the City of Chestermere, and as such is deemed to be an activity of the City of Chestermere. The personal information is collected under the authority of Section 33(1)(c)of the Freedom of Information and Protection of Privacy Act, and will be used only for the purpose identified.
